
Multimorbidity, the co-occurrence of two or more chronic conditions, affects the majority of older adults and is increasingly common in younger age groups in high-income countries. Fragmented disease-specific care models are ill-suited to patients with complex comorbidity, driving investment in integrated, person-centred care pathways. Deprescribing, shared decision-making, and life-goal aligned priority setting are central to multimorbidity management research.
