Polypharmacy, conventionally defined as concurrent use of five or more medications, is highly prevalent in older adults and associated with adverse drug reactions, drug-drug interactions, and non-adherence. Deprescribing, structured medication review, and pharmacist-led interventions are evidence-based approaches to reducing inappropriate medication burden. Clinical decision support tools and explicit criteria such as STOPP/START are widely used in practice.
