Portal hypertension, most commonly resulting from liver cirrhosis, is the driver of major complications including variceal haemorrhage, ascites, and hepatic encephalopathy. Non-selective beta-blockers and endoscopic variceal ligation are cornerstones of primary prophylaxis; TIPS placement addresses refractory complications. Novel approaches targeting splanchnic circulation and liver fibrosis are under active investigation.
